Order a Sleep Study Online

 

If you are a physician, or you are contacting us from a physician's office,
you can now use our Sleep Study "E-Order" Form for ordering sleep studies.

Once you click on the "E-Form" nearest your lab, you can:

1. Download the form to your computer by clicking "Save" and complete the order form from your own computer; or

2. Complete the "E-Form" online by clicking "Open." Once you complete the form, print the order and then fax it to your local Sleep Center. 

If you decide to save the "E-Form," it's a good idea to save the file using the name of your patient, in this format: LastName-FirstName.doc

It is important to fax and do not email the form in order to remain HIPPA compliant.
Keep a printed copy of the order form for your files.
